Dental Services and Procedures:

Restorative Dentistry that includes:

Crowns,

Bridges,

Partials,

Dentures,

Fillings.

Cosmetic Dentistry:

Veneers,

Bonding,

Zoom Whitening, and Tres White whitening strips.

Preventative:

Routine cleanings,

Treatment of gum disease,

Sealants,

Extractions including wisdom teeth, Individualized oral hygiene instruction 

Oral cancer screening.

Cancelation Policy:

If you do not show up for your appointment or give less than 24 hours notice you may be charged a $25 cancelation fee.

Financial Policies:

We accept check, credit card and cash. Balances that are 90 days overdue are subject to interest charges of 18%. Uncollectable accounts will be sent to the credit bureau and/or small claims court.

As a courtesy to our patients we still accept your insurance for the estimated portion of what they will cover and ask the patient to pay the difference at the time of service. Please note that your insurance is a contract between you and your insurance company and that every policy is different. As such, we can only estimate their coverage and you will be responsible for the difference. The only exceptions are if you have a policy that will not pay our office directly or if you have a policy that has a provider list that we do not participate in then you will need to pay in full and be reimbursed by your insurance company.

We do not accept Medicaid or Medicare.
